Tumor Mass Come Back In The Middle Of Rituxan Bendamustine Therapy?

my Dad has been in Rituxan Bendamustine therapy and showed good response in the first infusion, but after second infusion seems the tumor mass back to be bigger. Does it common in the process? or is this sign of refractory?

Im curious. Most of you are from The U.S.A. I'm from South-Africa and I believe our medical costs works differently. Does americans have to pay privately for medical costs or does the government cover a pirtion or all?
