Rituximab Maintenance Therapy After R-CHOP Or Not??? Has Anyone Chosen Not To And Regretted It?

A MyLymphomaTeam Member asked a question 💭
Tallahassee, FL

Finished R-CHOP for FL in December and am feeling great, except really achy joints. Since I had a complete remission after 4 rounds of chemo, my doctor is giving me the option of doing maintenance therapy (Rituximab) every 8 weeks for 2 years OR skipping that and just doing regular scans so he can monitor me. I have gone down hours of trial results rabbit holes, and it's a mixed bag. Half say it makes a difference and half say it is not worth the toxicity in low-risk patients. (I was just… read more

I tried maintenance and it came back during it. Since then I only use rituxan when it’s active and coming out of remission

KristiH,
Hi! Congratulations on your remission. I have/had DLBCL stage IV and had treatments of RCHOP chemo. Some people with DLBCL have follow up rituxin. I was never offered it. I've been in remission for 3 1/2years. My suggestion is if your Dr won't give you a medical opinion in regards to this decision get a 2nd opinion from another oncologist. Good luck on your decision. I had RCHOP & radiation and was clean for 2 yr 8 mon and the Large B Cell Lymphoma returned. It was discovered at my 6 month follow up CAT Scan. Does your insurance cover the Rituximab? What's the % of cancer returning? With cancer relapse, I had chemo with Rituximab immunotherapy which as an injection in my stomach- no big deal anymore compared to the ultimate RICE & BEAM CHEMO that led to an auto stem cell transplant.

Thank you Kelly! That is good advice. Love my oncologist but not loving his vagueness on this issue, even though I understand that the data is mixed and there is probably no wrong answer. I just hate making decisions, Lol. Thanks for the kind words, and congrats to you on your 3.5 year remission!

Deciding on Rituximab maintenance therapy after R-CHOP for follicular lymphoma (FL) can be challenging, especially when you've achieved complete remission. Maintenance therapy may help prolong remission, but it also comes with potential side effects, including joint pain, low white blood cell counts, and infusion reactions, which you've already experienced. Since your doctor suggests you might be fine without it, you could weigh the risks of toxicity against the benefits of close monitoring. Discuss your concerns with your healthcare team to make the best choice for your situation.
